Please protect your privacy
These forms request sensitive personal and health information, including medical history and payment details. Do not email completed forms and do not submit them through any website form. Email is not a secure way to transmit protected health information. Return your completed paperwork in one of the ways listed below.
- Bring it in person to your first appointment at our Humble office.
- Ask about our secure portal by calling the office before you send anything.
- Provide payment details by phone — never enter card numbers into a web form or email.

How to complete your paperwork
Step 1
Download the correct packet — adult for yourself, or child if you are a parent or legal guardian enrolling a minor.
Step 2
Print and complete every page, including the consent and privacy acknowledgment forms. Sign where indicated.
Step 3
Bring the completed forms to your first appointment, or call (832) 576-5538 to arrange secure return.
The Social Security Number field is required only if you are filing with insurance. If you are unable to print the forms, we will provide paper copies at your first appointment. Please arrive 15 minutes early to complete any remaining paperwork.
